The INDIANA CARPENTERS WELFARE FUND is a standard-design dental plan, paying a 75% Coinsurance Benefit for all eligible services after the Member satisfies an annual deductible of $100.

This plan does not provide any Orthodontia coverage, but INDIANA CARPENTERS WELFARE FUND members are entitled to the Maverest Fee Schedule for Orthodontia Services.

INDIANA CARPENTERS WELFARE FUND insureds must present an ID Card similar to the example provided at the bottom of this bulletin to be eligible for Maverest discounts.

IMPORTANT: Because of the INDIANA CARPENTERS WELFARE FUND'S membership structure, there may be isolated situations where INDIANA CARPENTERS WELFARE FUND members will not have medical or dental coverage in effect at time of service. Because of those possible lapses in coverage, ALWAYS VERIFY ELIGIBILITY AND BENEFITS. ALSO, during those possible lapses in coverage eligibility INDIANA CARPENTERS WELFARE FUND members are still entitled to the Maverest Fee Schedule for services rendered.
